How to

Fill in the online form

  1. Create an account with the Nurse Portal
  2. Once you’re logged into your account, click on “Application” and “Endorsement Application”
  3. Follow the online application process
  4. You will need to provide the following:
    • Name, location and year of graduation of your basic school of nursing.
    • State, year and license number of original license by examination.
    • State of current license and license number.
    • Dates of employment and position(s) held for last two (2) nursing employers. 
    • All U.S. Member Boards in which you have EVER been licensed.
    • Social Security number issued by U.S. Social Security Administration.
    • Verification from original state of license and, if different, last state practiced.
  5. Fill in the Verification of Current Status Form, available here, and send it to the State in which you are currently or were last employed.
