How to
Fill in the online form
- Create an account with the Nurse Portal
- Complete the account registration
- Click on “apply for license” at the top right corner of the first box “Your Licenses with Wyoming”
- Under “CNA Endorsement Application” click on “CNA”
- Complete the application
- Pay the fees and submit your application with all supporting documentation as indicated in the form
- Enclose a copy of your current CNA certification Fax or mail the documents (do not email!) – see below for details
- Mail to the WSBN two (2) completed FBI Fingerprint cards (see above) – you must include “CRIMINAL BACKGROUND CHECK” as reference
- Mail or fax:
- A copy of your social security card
- Another form of lawful presence (driver’s license, birth certificate, passport, or other items listed on the application)
