How to keep your license on the Connecticut CNA Registry

Stay Active

The certification period is for 24 months, and you must renew every 2 years by providing verification that you have worked as a nurse aide or in a position providing nursing related services for a minimum of eight paid hours during the 24-month time frame.

Renew

To renew your application, you need to download the Connecticut Nurse Aide Employment Verification Form and complete the Nurse Aide section. Afterward, contact your employer to complete the employer section. The completed form then needs to be returned to the Nurse Aide Registry by email (dph.nurseaide@ct.gov), fax (860 707-1983), or sent by mail to the address listed at the bottom of the form.

There is no fee to renew your certification.

Reactivate your expired license

If your certification has lapsed but you have been working as a nurse aide or provided nursing related services during the last 24 months, you can renew your certification using the Connecticut Nurse Aide Employment Verification Form (see section above).

If your certification has lapsed and you haven’t been working in a nursing related capacity during the last 24 months, you will not be eligible for renewal. In that case, you will need to retake the Connecticut nurse aide competency exam, which is administered by Prometric. To register for the exam, you can submit an online application or paper application (choose Route 4).
