Requirements to get on the Utah CNA Registry

Fees

The application process is free of charge.

Minimum Age

Some training schools will require you to be at least 17 years old to apply.

Minimum Education

Training school may require to have a high school diploma or a GED.

Training Requirements

To qualify you must train with a state-approved training program that will provide you with at least 100 hours of supervised and documented training by a licensed nurse.

Competency Test

To be certified on the Utah Nurse Aide Registry, you must take both a skill test and a knowledge test. Upon completion of a training program or by means of reciprocity, you will be assigned your own personal portal and username and password with TMU. You will need your username and password when you take your test.

You have 1 year from the date of completion of your training program to pass both portions of the exam.

Skill test: $55
Knowledge test: $35 – note the fee will increase to $55 on the 1st of July 2024.

Background Check

Where to apply

Once you have passed both tests, you will be automatically added to the Registry. You can review your profile on TMU. An official state certificate with wallet card will also be mailed out to you at the address in your TMU account.

Restrictions

You will not be able to practice if you are found guilty of abuse, neglect, or misappropriation.

Processing Time

2 – 3 business days from taking the tests.

How to keep your license on the Utah CNA Registry

Stay Active

The certification period is for 24 months, and you must renew every 2 years by providing verification that you have worked a minimum of 200 paid hours under the direction of a licensed nurse during the 24-month time frame.

Renew

You will receive an email and text message 90, 60, and 30 days prior to your expiration date to renew your certification. Renewal forms are available in your TMU account 90 days prior to the expiration date of your certification or can be accessed here.

Renewal Late Fee (1 – 30 days late): $20
Renewal Late Fee (2 – 6 months late): $40

Processing time: 7 – 10 business days.

Reactivate your expired license

If your certification expired more than 6 months ago but less than 24 months ago, you have not met the minimum 200 paid hours requirement, or you are within 90 days of expiration and have not met the 200 hours requirement, you must take a test under the Expired Certificate Application and pay the associated fee of $90. You must fill in this form to take the test.

If your certificate expired over 24 months ago, you will not be eligible to test under an Expired Certificate Application. You will need to retake a CNA course from one of the State-approved training programs.
