How to
Fill in the online application:
- Log in to Prometric here
- Fill in “Section 1. Candidate Information”
- In “Section 2. Certification Route” tick “Route 2. Reciprocity/CNA from another state”
- Fill in “Section 3. Employment Information” print a copy and have it signed by your current employer if the state in which you are currently certified does not provide expiration dates on its registry – and provide proof of employment. This must be at least on facility letterhead from your employer. It must indicate that you have worked seven hours for pay in the past 24 months. The letter must state that you have performed nurse aide duties and must be signed by your supervisor
- In “Section 5. Fees” tick “Application Processing Fee – No exam required” under “Reciprocity and NYS RNs/LPNs Routes 2 and 4”, and tick the box of the preferred method of payment
- Review and agree to “Section 6. Applicant’s Affidavit”
- Upload or send your supporting documents via Mail or Fax. Supporting documents include a copy of your current out-of-state nurse aide certificate(s) (and any proof of change of name if applicable), and where applicable proof from employer as set out above.

- Fill in the “Candidate Information”
- In “Certification Option/Eligibility” tick “Route 2. Reciprocity/CNA from Another State”
- In “Exam Selection and Processing/Exam Fees” tick “Reciprocity/CNA from Another State and NYS RNs and LPNs Application Processing” and write “50” in the “Total Fee”
- Review, sign and date the “Applicant’s Affidavit and Candidate Release Statement”
- Fill in the “Payment Form”
- Include a copy of your current out-of-state nurse aide certificate(s) – If the name on your nurse aide certificate is different from your current name, submit proof of name change (i.e., marriage license)
- Note: If the state in which you are currently certified does not provide expiration dates on its registry, you must provide proof of employment.
