Training & Certification Requirements in California

Before starting your training, it's important to understand exactly what California requires to become a Certified Nursing Assistant (CNA), officially titled Nurse Assistant Certified in California. Meeting these standards ensures you're eligible to test, be placed on the state registry, and work in approved healthcare settings.

Eligibility

  • Age requirement: Candidates must be at least 16 years old to enroll in a California-approved CNA training program.
  • Program enrollment (NATCEP): You must complete a California Department of Public Health (CDPH)–approved Nurse Assistant Training and Competency Evaluation Program (NATCEP). Programs are offered through community colleges, adult schools, regional occupational programs (ROPs), and licensed healthcare facilities.
  • Background check: All candidates must complete a criminal background check through California DOJ and FBI Live Scan. Individuals with disqualifying convictions or substantiated findings of abuse, neglect, or misappropriation are not eligible for certification or registry placement.
  • Good standing: Employers must verify CNA registry status prior to employment. Individuals listed on the Nurse Aide Registry with disqualifying findings may not work in long-term care settings.

Training Hours & Clinicals

California requires a minimum of 160 total hours of nurse assistant training, which exceeds the federal minimum. This includes at least 60 hours of classroom (theory) instruction and 100 hours of supervised clinical training in a licensed skilled nursing facility. The curriculum covers required OBRA topics such as communication, infection control, residents' rights, safety and emergency procedures, basic nursing skills, personal care, and dementia-related care including Alzheimer's disease.

Before providing any direct resident care, trainees must complete at least 16 hours of instruction covering fundamental safety and care principles. Medicare- or Medicaid-certified nursing facilities may employ CNA trainees for up to 120 days while training and testing requirements are completed; beyond this period, individuals must be fully certified and listed on the California Nurse Aide Registry.

Competency Exam (Knowledge & Skills)

After completing training, candidates must pass California's state-approved competency examination administered by Credentia or D&SDT-Headmaster on behalf of CDPH. The exam includes:

  • Knowledge test: A 70-question exam consisting of 60 multiple-choice questions and 10 reading comprehension questions covering infection control, safety, resident rights, communication, and basic nursing care. Candidates have 120 minutes (2 hours) to complete the exam. An oral version is available for eligible candidates.
  • Skills evaluation: Demonstration of five randomly selected nurse assistant skills within 25 minutes, including mandatory hand hygiene and required critical steps. All critical elements must be performed correctly to pass.

Candidates have up to three attempts to pass each component within two years of completing training. If a candidate fails one component, only the failed portion must be retaken. Candidates who do not pass within the allowed attempts must complete a new CDPH-approved training program before retesting.

Placement on the Registry & Working as a CNA

Once both exam components are successfully passed, your name is added to the California Nurse Aide Registry, maintained by the California Department of Public Health. Credentia or D&SDT-Headmaster transmits test results electronically to CDPH. You may then work as a CNA in skilled nursing facilities, hospitals, home health agencies, and other licensed healthcare settings throughout California.

Renewal

California CNA certification must be renewed every 24 months. To remain active on the registry, CNAs must complete 48 hours of approved in-service or continuing education during each two-year renewal period. A minimum of 12 hours must be completed in each year of the two-year period, and a maximum of 24 of the total 48 hours may be completed online through CDPH-approved providers. Failure to complete continuing education or renew on time may result in inactive or expired status.

Reciprocity

California offers certification by equivalency for CNAs certified in other U.S. states. Applicants must hold an active certification in good standing and provide verification that their training and competency evaluation met federal OBRA standards. Additional documentation, background checks, or testing may be required before placement on the California Nurse Aide Registry.

Costs

California CNA exam fees are set by Credentia and D&SDT-Headmaster and include separate charges for the knowledge test and skills evaluation. Fees are paid at the time of scheduling. Under federal law, Medicare- or Medicaid-certified nursing facilities must pay for or reimburse CNA training and testing costs for individuals they employ or have offered employment to, provided the individual is hired within 12 months of completing training.
