How to

Access and fill in the online form:

  1. Log in or create an account with Prometric.
  2. Click on new application form and select “Louisiana Certified Nursing Assistant Application”.
  3. Fill in “Candidate Information”.
  4. In “Certification Option/Eligibility” tick route 8 and fill in the required information regarding your CNA active certificate in other states.
  5. Tick “Reciprocity Application Processing Fee” in “Exam Selection and Processing/exam fees”.
  6. Choose your payment method.
  7. Review and accept the affidavit.
  8. Upload a copy of your social security card and a copy of your official Louisiana identification, such as a driver’s license, identification card Approval Letter received from the State and attach it under the Supporting Documents Section before you submit your online application.
